HepBest is a prodrug; TAF is a phosphonoamidate of tenofovir.

TAF has lipophilic cell impermeability effect, which percolates into primary hepatocytes by undergoing passive diffusion and also by hepatic intake Hepbest  tablet transporters like OATP1B1 & OATP1B3

Inside the cells, TAF is changed into tenofovir Hepbest  by hydrolysis with the help of carboxylesterase present in hepatocytes cells

Again tenofovir Hepbest  tablet is phosphorylated into pharmacologically active metabolite tenofovir diphosphate by cellular kinase

This metabolite involved in the inhibition of viral replication Hepbest   by insertion into viral DNA by reverse transcriptase, concluded in termination of viral DNA chain

The maximum peak concentration level occurs within 0.48hours after an oral administration

The effect of high fat meal corresponding to fasted state occurs in the range of 1.51 hours & 1.81 hours

Side effects caused during therapy

 Hepbest   is a substrate of P-gp & breast cancer resistance protein; P-gp & BCRP strong inhibitors are concomitant withHepBest tablets which may  Hepbest  tabletcause altering the TAF absorption

 Hepbest  tablett concurrently used with P-gp inducers leads to decrease the TAF absorption, which concluded in depletion of plasma concentration and causes low therapeutic effect

 Hepbest   co administered with moderate P-gp & BCRP inhibitors causes elevation of absorption and plasma concentration of tenofovir Alafenamide

Mostly tenofovir is excreted via urine, through kidney; combination of  Hepbest  tablet with the drugs which is reducing the renal function which may causes elevation of tenofovir concentration & renally excreted drugs causes some adverse effects

Examples: Acyclovir, valaciclovir, amino glycosides, Ganciclovir, cidofovir, NSAIDS

HepBest tablet concomitant with anti-convulsants drugs like phenytoin, Phenobarbital, carbamazepine causes decreasing the concentration of TAF

HepBest with anti-mycobacterials like rifabutin, rifampin or rifapentine may leads to decrease the TAF

Tenofovir Alafenamide combined with herbal product like st. Johns wort causes decreasing in serum concentration of tenofovir Alafenamide

Reason for withhold the therapy

No contraindication occurred during therapy

Some anaphylactic reactions may occurs if patient are contraindicated to the component present in HepBest tablet

Before starting the therapy, patients should be investigate for HIV-1 infection, in case of HIV-1 infection present, tenofovir Alafenamide should not be used alone

Creatinine clearance

Urine glucose

Urine protein

How to take HepBest tablets


 Hepbest  tablet  is a single dose regimen; it must be taken as once a day

 Hepbest   are administered orally with whole some water, it should not be crushed, chew, do not break the tablet

 Hepbest  tablet should be taken with food

It is a prescription medicine, used only under the guidance of medical practitioner

Precaution takes place during the therapy

After completion of therapy, some adverse effects may occur to overcome the problems some precaution may take place;

Conditions like;

Lactic acidosis or serious hepatomegaly with steatosis:

Obesity and extended nucleoside exposure are the risk factors of this condition; if this adverse effect occurs then the therapy with  Hepbest  tablet should be postponed

Aggravation of renal impairment occurs; check Serum creatinine, Serum phosphorus, Creatinine clearance, Urine glucose, Urine protein frequently Hepbest to avoid these problems, otherwise stop the treatment using with  Hepbest   to escape from renal related adverse effects

Storage

 Hepbest  container should be stored at room temperature at below 30oC (86oF)

1.Pregnancy

Missed dosage
